كل علاجٍ نفسيٍّ يبدأ من فكرة. والعلاج الجدلي السلوكي بدأ من تناقض.Every psychotherapy begins from an idea. Dialectical behavior therapy began from a contradiction.
تخيّل أن أحدًا جلس أمامك وقال لك جملتين، واحدة بعد الأخرى مباشرة:Imagine someone sat in front of you and said two sentences, one straight after the other:
«أنا متقبّلك تمامًا كما أنت. لا شيء فيك يحتاج إلى إصلاح.»“I accept you exactly as you are. There is nothing in you that needs fixing.”
ثم: «ولازم تتغيّر. الطريقة اللي بتعيش بيها دلوقتي بتوجعك.»Then: “And you must change. The way you are living right now is hurting you.”
أول ما سيفعله عقلك هو أن يطلب منه أن يختار واحدة. لأن الجملتين لا يمكن أن تكونا صحيحتين معًا.The first thing your mind will do is ask him to pick one. Because those two sentences cannot both be true.
وهذا بالضبط ما رفضت امرأةٌ في سياتل أن تفعله. ومن رفضها وُلد علاجٌ كامل.And that is precisely what a woman in Seattle refused to do. Out of her refusal, an entire therapy was born.
الكلمة التي يمرّ عليها الناس بسرعةThe Word People Hurry Past
أغلب من سمعوا عن الـDBT سمعوا عنه بوصفه «مهارات». تمارين، وأوراق عمل، وحاجات تعملها لما تتعصّب. وهذا جزءٌ حقيقي منه، لكنه ليس قلبه.Most people who have heard of DBT heard of it as “skills.” Exercises, worksheets, things you do when you get angry. That is a real part of it, but it is not its heart.
لاحظ أن اسم العلاج لا يبدأ بالمهارات. يبدأ بكلمةٍ يمرّ عليها الناس بسرعة لأنها تبدو أكاديمية وثقيلة: الجدلي.Notice that the name of the therapy does not begin with the skills. It begins with a word people hurry past because it sounds academic and heavy: dialectical.
والجدل هنا ليس المناظرة ولا المحاجّة. وليس حلًا وسطًا، ولا «نص نص»، ولا أن نلتقي في المنتصف فيخسر كلٌّ منا نصف ما يقوله.And dialectics here is not debate or argument. It is not a compromise, not “half and half,” not meeting in the middle so that each of us loses half of what he was saying.
الجدل يقول شيئًا أصعب: أن فكرتين متناقضتين قد تكونان صحيحتين معًا، وأن الحقيقة أكبر من أن تتّسع لها واحدة.Dialectics says something harder: that two contradictory ideas may both be true, and that the truth is too large to fit inside either one of them.
«أنا عملت أحسن اللي أقدر عليه.» صح.
«ولازم أعمل أحسن من كده.» صح كمان.“I did the best I could.” True.
“And I have to do better than that.” Also true.
عقلك يريد أن يختار. يريد أن يعرف أيّهما الحقيقية، لأن الاختيار يريحه. والحياة لا تمنحنا هذه الرفاهية كثيرًا.Your mind wants to choose. It wants to know which one is the real one, because choosing is a relief. Life does not often grant us that luxury.
الحياة الناضجة ليست حياةً بلا تناقض. هي حياةٌ تعرف كيف تحمله دون أن ينهار صاحبها.A mature life is not a life without contradiction. It is a life that knows how to carry it without the one carrying it collapsing.
كيف اكتُشف هذا — مع أصعب الحالاتHow This Was Discovered — With the Hardest Cases
في السبعينيات، في جامعة واشنطن بمدينة سياتل، كانت طبيبةٌ نفسية اسمها مارشا لينهان تشتغل مع فئةٍ كان كثيرٌ من الأطباء وقتها يتجنّبونها: أشخاصٌ يحاولون إنهاء حياتهم مرةً بعد مرة، وقيل عنهم إنهم «لا يستجيبون للعلاج».In the 1970s, at the University of Washington in Seattle, a psychologist named Marsha Linehan was working with a group many clinicians of the time avoided: people who tried to end their lives again and again, of whom it was said that they “do not respond to treatment.”
بدأت بما كان متاحًا: العلاج المعرفي السلوكي كما هو. اتعلّم تغيّر أفكارك، غيّر سلوكك، جرّب طريقةً أخرى. وكانت النتيجة غريبة — كان المرضى ينسحبون. بعضهم يترك العلاج تمامًا.She began with what was available: cognitive behavioral therapy as it stood. Learn to change your thoughts, change your behavior, try another way. And the result was strange — her patients withdrew. Some left therapy altogether.
لأنهم كانوا يسمعون في كل محاولةٍ لتغييرهم رسالةً واحدة: إنت اللي غلط. إنت مش شايفني. لو كنت فاهم حجم اللي أنا فيه ما كنتش قلت لي «جرّبي تفكّري بطريقة تانية».Because in every attempt to change them they heard one message: you are the thing that is wrong. You do not see me. If you understood the size of what I am in, you would not tell me to “try thinking about it differently.”
فجرّبت العكس تمامًا: التقبّل الكامل. اجلس معهم، اسمع، لا تضغط، لا تطلب شيئًا. وكانت النتيجة أسوأ — شعروا بأنها استسلمت لهم. يعني الوجع ده هيفضل كده؟ إنتي كمان مش شايفة إن فيه مخرج؟So she tried the exact opposite: full acceptance. Sit with them, listen, do not push, do not ask for anything. And the result was worse — they felt she had given up on them. So this pain is simply going to stay? You too cannot see a way out?
ولم يكن الحلّ أن تختار. كان أن تحمل الاثنين: في الغرفة نفسها، وفي الجلسة نفسها، وأحيانًا في الجملة الواحدة. «أنا شايف إن ده أصعب حاجة مريت بيها، وإن ده كان أقصى اللي قدرت عليه — وتعال نشتغل على حاجة تانية الأسبوع ده.»And the solution was not to choose. It was to hold both: in the same room, in the same session, sometimes within a single sentence. “I can see that this is the hardest thing you have been through, and that it was the most you could manage — and let us work on something different this week.”
ومارشا لينهان لم تكن تتحدث من خارج التجربة. بعد أكثر من ثلاثين سنة من بناء هذا العلاج، كشفت للملأ سنة ٢٠١١ أنها هي نفسها قضت جزءًا من شبابها في جناحٍ مغلق للحالات الميؤوس منها، وأن العلاج الذي طوّرته هو العلاج الذي كانت تحتاجه ولم يكن موجودًا. حكايتها كاملة هنا.And Marsha Linehan was not speaking from outside the experience. More than thirty years after building this therapy, she revealed publicly in 2011 that she herself had spent part of her youth in a locked ward for hopeless cases, and that the therapy she developed was the therapy she had needed and which did not exist. أربع عائلات من المهاراتFour Families of Skills
وفوق هذا الأساس تقف المهارات، وهي أربع وحدات:On top of that foundation stand the skills, in four modules:
اليقظة الذهنية. أن تلاحظ ما يجري بداخلك وأنت فيه، لا بعد أن ينتهي بثلاثة أيام. أن تعرف الفرق بين «أنا حاسس إني فاشل» و«أنا فاشل».Mindfulness. Noticing what is happening inside you while it is happening, not three days after it ends. Knowing the difference between “I feel like a failure” and “I am a failure.”
تحمّل الضيق. ما تفعله في الساعة التي لا يمكن فيها إصلاح أي شيء — ساعة الأزمة — حتى تعبرها دون أن تزيد الأمور سوءًا. هنا تسكن مهارة التقبّل الجذري: أن تكفّ عن محاربة ما وقع فعلًا، لا لأنه عادل، بل لأن الحرب معه تكلّفك مرتين.Distress tolerance. What you do in the hour when nothing can be fixed — the hour of crisis — so that you cross it without making things worse. This is where radical acceptance lives: ceasing to fight what has already happened, not because it is just, but because fighting it costs you twice.
تنظيم المشاعر. أن تفهم ماذا تفعل المشاعر ولماذا جاءت، وأن تقلّل سطوتها بدل أن تتحكّم فيك. ومنها مهارة الفعل العكسي: أن تحدّد ما يدفعك إليه الشعور — الانسحاب، الصمت، التأجيل — ثم تفعل عكسه بالكامل.Emotion regulation. Understanding what emotions do and why they came, and loosening their grip instead of being run by them. Among these is opposite action: identifying what the feeling pushes you toward — withdrawal, silence, postponement — and then doing the complete opposite.
العلاقات الفعّالة. أن تطلب احتياجك، وتقول «لأ» حين تعني «لأ»، وتخرج من المحادثة وقد حافظت على العلاقة وعلى احترامك لنفسك معًا — وهما شيئان كثيرًا ما نضحّي بأحدهما من أجل الآخر.Interpersonal effectiveness. Asking for what you need, saying “no” when you mean no, and leaving a conversation having kept both the relationship and your self-respect — two things we very often sacrifice one of for the other.
واليقظة الذهنية ليست الأولى بالصدفة. هي الوحيدة التي تُعاد بين كل وحدةٍ والتي تليها، لأن المهارات الثلاث الباقية لا تعمل إن لم يكن صاحبها حاضرًا بما يكفي ليلاحظ أنه يحتاجها.And mindfulness is not first by accident. It is the only module repeated between each of the others, because the remaining three do not work unless the person is present enough to notice that he needs them.
الفرق بين النصيحة والمهارةThe Difference Between Advice and a Skill
كثيرون يسمعون هذا الكلام فيقولون: طيب وإيه الجديد؟ ده كلام معروف. اهدى، خد نفسك، اتكلم بهدوء.Many people hear all this and say: so what is new? This is common knowledge. Calm down, take a breath, speak gently.
والفرق كبير، وهو الفرق بين النصيحة والمهارة. النصيحة معلومة تدخل من الأذن وتستقرّ في الرأس. والمهارة شيءٌ يتدرّب عليه الجسد بالتكرار حتى يعمل في اللحظة التي يتوقّف فيها الرأس عن العمل.The difference is large, and it is the difference between advice and a skill. Advice is information that enters the ear and settles in the head. A skill is something the body rehearses by repetition until it works in the moment when the head stops working.
لا أحد يتعلّم العوم وهو يغرق. يتعلّمه في المياه الضحلة، مئة مرة، وهو غير محتاجٍ إليه أصلًا — حتى يجيء اليوم الذي يحتاجه فيه فيجد الجسد يعرف.Nobody learns to swim while drowning. You learn in shallow water, a hundred times, when you do not need it at all — so that on the day you do need it, you find that your body already knows.
ولهذا فإن جروب المهارات في الـDBT يشبه الفصل أكثر مما يشبه الجلسة: فيه منهج، وفيه واجب بين الأسبوع والذي يليه. لأن المهارة التي لا تُكرَّر تبقى معلومةً جميلة لا تنفع وقت الحاجة.This is why a DBT skills group resembles a class more than a therapy session: there is a curriculum, and there is homework between one week and the next. Because a skill that is not repeated stays a pretty piece of information that is of no use when it is needed.
ليس برنامجًا من جلسةNot a One-Session Programme
الـDBT في صورته القياسية ليس جلسةً أسبوعية. هو أربعة أجزاء تعمل معًا: جلسة فردية، وجروب للمهارات، وإمكانية التواصل مع المعالج وقت الأزمة لا بعد أسبوع، ثم جزءٌ رابع غريب على من يسمعه لأول مرة: فريقٌ يجتمع فيه المعالجون أنفسهم كل أسبوع.Standard DBT is not a weekly session. It is four parts working together: individual therapy, a skills group, the ability to reach your therapist in a crisis rather than a week later, and then a fourth part that sounds odd to anyone hearing it for the first time: a team in which the therapists themselves meet every week.
والجزء الرابع هذا ليس ترفًا إداريًا. العمل مع ألمٍ بهذا الحجم يُنهك المعالج أيضًا، ويجعله ينزلق بهدوء إلى أحد الطرفين: إما أن يقسو ويطالب بتغييرٍ لا يحتمله صاحبه، وإما أن يرقّ ويستسلم للعجز معه. الفريق موجودٌ ليعيده إلى منتصف الحبل.And that fourth part is not administrative luxury. Working with pain of that size exhausts the therapist too, and makes him slide quietly toward one of the two poles: either hardening and demanding a change the person cannot bear, or softening and surrendering to the helplessness alongside him. The team exists to pull him back to the middle of the rope.
لمن هذا العلاج — وما حدودهWho It Is For — and Where It Stops
بدأ الـDBT لاضطراب الشخصية الحدية وللسلوك الانتحاري المتكرر، وهناك في هذين تحديدًا أدلةٌ بحثية متراكمة على فاعليته، من تجاربَ عشوائية محكّمة ومن مراجعاتٍ جمعت نتائجها. ثم امتدّ بعد ذلك إلى مجالاتٍ أخرى — الإدمان، واضطرابات الأكل، والمراهقين — بدرجاتٍ مختلفة من قوة الدليل، والأمانة أن يُقال إن بعض هذه المجالات أقوى دليلًا من بعض.DBT began for borderline personality disorder and for recurrent suicidal behavior, and in those two specifically there is accumulated research evidence for its effectiveness, from randomized controlled trials and from reviews pooling their results. It later extended to other areas — substance use, eating disorders, adolescents — with varying strengths of evidence, and honesty requires saying that some of those areas are better supported than others.
والأمانة تقتضي أيضًا ما هو أبعد: الـDBT ليس سحرًا، ولا يناسب كل إنسان، ولا هو الاختيار الأول لكل اضطراب. وهو بطيء — البرنامج القياسي سنةٌ كاملة. ومن يَعِدك بتحوّلٍ في ستّ جلسات لا يبيعك DBT، يبيعك شيئًا آخر باسمه.And honesty requires going further: DBT is not magic, it does not suit everyone, and it is not the first choice for every disorder. It is slow — the standard programme runs a full year. Anyone promising you a transformation in six sessions is not selling you DBT; he is selling you something else under its name.
الجملة التي اختارتهاThe Sentence She Chose
وكان بوسع مارشا لينهان أن تختار أي شعارٍ برّاق لتسمّي به هدف هذا العلاج كله. لم تسمّه الشفاء، ولا اختفاء الأعراض، ولا السعادة.Marsha Linehan could have chosen any glittering slogan to name the goal of this entire therapy. She did not call it cure, nor the disappearance of symptoms, nor happiness.
سمّته: حياة تستحقّ أن تُعاش.She called it: a life worth living.
لأن الهدف ليس أن تكفّ عن الوجع. بعض الوجع لن يُحلّ، والفقد لا يُعاد، والماضي لا يُعدَّل. الهدف أن يصير للوجع اتجاه، وأن تبقى واقفًا في الاتجاه الذي يهمّك وأنت تحمله.Because the goal is not that you stop hurting. Some pain will not be solved, loss is not returned, the past is not amended. The goal is that the pain acquires a direction — and that you keep standing in the direction that matters to you while carrying it.
ارجع الآن إلى الجملتين اللتين بدأنا بهما. «أنا متقبّلك تمامًا كما أنت» و«ولازم تتغيّر». لم تعودا متناقضتين، أليس كذلك؟ لقد صارتا الطريقة الوحيدة التي يمكن بها أن يُقال الاثنان لإنسانٍ موجوعٍ دون أن يُهان بأحدهما.Now go back to the two sentences we began with. “I accept you exactly as you are” and “and you must change.” They are no longer contradictory, are they? They have become the only way both can be said to a person in pain without one of them humiliating him.
