With time, and with years of working with human pain, I began to revisit one of the most entrenched ideas in how we treat survivors of psychological trauma: the belief that a person who has suffered severely must necessarily need more empathy, more holding, and more closeness to their inner fragility.
As if suffering had stripped them of everything and left behind nothing but a wound waiting for someone to pat it.
And I kept saying to myself: wait a moment. How did this survivor live through all those years? Who said the person sitting in front of me is merely a fragile human being? And who gave me the right to treat their defenses as a problem, before I understand that those defenses may have been the only reason they stayed alive?
The survivor had the strength to survive, or at least possessed tools that allowed them to remain despite everything that happened. Perhaps I should see that latent strength, watch them draw some of it from me at times, and, much more often, help them see what they already possess.
Because the capacity to get through times of confrontation, and moments of human fragility, needs empathy as much as it needs strength.
القصص: ٣٥“We will strengthen your arm through your brother.” Strengthening the arm is a meaning worth pausing on. Support does not necessarily mean that I carry you because you cannot stand. It means that I add my strength to yours when you need it, without robbing you of the sense of your own capacity.
In the moments of advance and retreat, of wavering, of the collapse that looms and never completes, a person may need more than empathy and emotional validation. They need someone who knows when to come closer, when to stop, when to confront, and when their mere presence is enough.
And sometimes they need a therapist who is not afraid of their strength.
One of the biggest dilemmas I see in trauma treatment is excessive empathy, when it turns from a therapeutic tool into the only lens through which we see the client.
We treat trauma as if it left nothing behind but ruin, and we forget that the person carrying it spent years building tools for survival. Some of them conscious, some formed without their choosing, and some so costly it is hard to imagine.
And yet, those tools served a function.
A function whose price we may argue about today, but whose importance in the psychological history of its owner we cannot deny.
The tool may be excessive hardness, or a harsh independence that lets no one come near. It may be unprecedented success, severe discipline, and an astonishing capacity to achieve. And it may be emotional numbness, withdrawal, addictive behaviors, or a dependence on drugs to ease a pain its owner could not bear any other way.
I am not equating these responses here, nor describing addiction as a healthy strength, nor counting every psychological defense as an effective means of survival in the long run.
But I refuse to put a behavior on trial before we understand its function.
Before you ask why this person will not give up their defenses, ask first: what were those defenses protecting them from?
And before you count their hardness as an obstacle to treatment, perhaps you should understand the price they paid to build it.
Yes. Exactly as I am telling you.
Not every trauma, even the hardest and cruelest, ends in a person who is visibly broken, or unable to work, love, and achieve.
Some survivors conquer the world outside the therapy room. They build companies, achieve exceptional success, and carry responsibilities no one else could bear. They may become the people everyone turns to in a crisis, while no one knows the size of the battle still going on inside them.
Does this mean they have recovered? Certainly not.
And does their success mean the trauma made their greatness? Also no.
Perhaps they succeeded despite the trauma. Perhaps some of the coping strategies they developed contributed to that success. And perhaps the success itself was a way of fleeing a pain they did not want to face.
But it is a mistake to reduce every achievement to a pathological compensation, just as it is a mistake to reduce every hardness to a denial of feeling.
A human being is more complex than these comfortable reductions.
And the therapist who has memorized but not understood will fall into this trap easily.
He sees hardness and calls it resistance. He sees independence and calls it fear of dependence. He sees numbness and counts it as a refusal to connect with feelings. And he sees excessive success and decides it is merely a cover for a deep fragility.
Then he begins his great therapeutic project: demolishing the defenses.
As if he were standing before an old building he wants to renovate, so he grabs the pickaxe and starts breaking walls, before asking which of these walls is holding up the roof.
And sometimes the result is catastrophic.
Imagine someone who spent twenty years building their psychological armor. Every disappointment added a layer. Every threat taught them how to watch. Every harmful relationship made them more cautious, and every time they had to survive alone deepened their belief that showing need is dangerous.
Then comes the therapist, driven by good intentions, trying to get very close, to extract the admission of pain, to expose the vulnerability hiding behind all that hardness, believing that emotional exposure is the necessary gateway to healing.
But what if this closeness, in its timing and its manner, is itself what threatens the person's sense of safety?
What if the tears you are waiting for do not mean liberation to them, but an experience of losing control?
And what if the defenses you are trying to dismantle are the last thing they trust to protect them?
You are not asking them to take off a heavy jacket so they can be comfortable.
You may be asking them to skin themselves alive.
Imagine the pain.
To be asked to give up something that took years to build, before you know how you will live without it. To lose your old defenses without having any other tools to protect you when the fear returns.
And the cruelest part: that all of this happens at the hands of someone you came to for help.
Honestly, sometimes the pain caused by demolishing a person's safety system this way can be worse than the pain they walked into the therapy room trying to deal with.
Here empathy, once detached from understanding and expertise, becomes capable of real harm.
Not because empathy is wrong, but because empathy alone is not enough.
Not every therapeutic closeness is appropriate, not every emotional exposure is progress, and not every psychological defense is an illness to be removed.
And one of the most dangerous things a therapist can do is to make the client's fragility the center of their therapeutic identity.
To overprotect them from confrontation, to treat every pain as something unbearable, or to send them, even indirectly, a message that says: you are weaker than you think, and I am here to protect you from yourself and from the world.
And here we face a cruel paradox.
We want to give the survivor emotional validation, yet we may fall into invalidating their capacity.
We acknowledge their suffering, but we do not acknowledge what they did to remain.
We see the wound clearly, and fail to see the human being who carried it all these years.
And I believe one of the most dangerous forms of invalidation is the one that wears the clothes of empathy.
That I see in you only the pain, while for years you have been trying to tell me that you are more than this pain.
On the other hand, I do not want talk of strength to turn into a harsh discourse that demands survivors hold firm, or burdens them with the responsibility of recovery simply because they managed to stay alive.
Survival does not always mean the ability to heal without help, and a person's continuing to live does not mean their tools were sufficient or sound, or that they now have the capacity to change them.
There are survivors who never had the chance to build strong armor in the first place. And there are people whom trauma has drained to the point where the simplest demands of life have become an enormous burden.
And respecting strength does not mean inventing it when its owner cannot feel it.
I am talking about the necessity of seeing the whole person, not about forcing them to put on a new heroism.
I do not want to swap the image of the victim for the image of the hero who is not allowed to fall.
Both images can rob a person of their freedom to be themselves.
And here appears one of the hardest tasks in psychotherapy: how do we respect the tools of survival, and at the same time help their owner discover that some of them no longer suit the life they want?
How do I tell someone that their hardness once helped them survive, without leaving them a prisoner of that hardness forever?
How do I respect numbness as an understandable response to threat, and at the same time help them recover their capacity to feel?
And how do I understand the function addiction served in easing suffering, without justifying its continuation or ignoring its dangers?
This is the real clinical dilemma.
Some of the weapons we use in war become a burden when we try to live in peace.
But what do you do with someone who is not yet convinced that the war is over?
Do you take away their weapon and demand that they feel safe?
Do you mock their armor and tell them it is no longer necessary?
Or do you first help them tell the difference between real danger and anticipated danger, between the past that has ended and the threat that may still be present?
And perhaps, once they acquire other tools and experience safety again and again, they will be able to set some of their weapons aside of their own will.
Not because those weapons were worthless, but because they are no longer the only means available to them.
This is exactly where the value of expertise appears.
The expert therapist is not afraid of the client's defenses, and does not go to battle with them to prove his authority. He does not rush to break through them, nor does he sanctify them to the point of preventing their change.
He understands their function, weighs their cost, and works with the client to build more flexible alternatives.
He knows that confrontation may be necessary, and that postponing it is sometimes more professional than insisting on it.
And he knows that validation and change are not adversaries.
And that, to me, is the heart of the dialectical idea in therapy.
You did what you could to survive, and some of what you did is now hurting you.
Both sentences are true.
And I do not need to cancel one of them in order to work with the other.
And from here I return to “We will strengthen your arm through your brother.”
I see in it a human meaning beyond consolation: to be beside you in the moment of helplessness without defining you by it, and to help you recover your capacity without pretending that I am its only source.
The strength that helped you survive may be one of the resources we will need in recovery, once we understand it, separate what is useful in it from what has become costly, and give you the freedom to use other tools.
I believe in the human brain's capacity to learn, reorganize, and change, in light of what we know about neuroplasticity. But this belief does not mean the brain repairs all its wounds automatically, or that recovery is guaranteed simply by summoning inner strength.
The capacity for change is there, but circumstances, relationships, and the right therapeutic expertise can be decisive in making that change possible.
And so I return to the first sentence:
“The more severe you're suffering, the more you need an expert and not a nice person.”
I do not want a therapist without kindness. Kindness is an indispensable part of the human relationship.
But I refuse to let kindness become a substitute for competence, to let the wish to hold the client turn into an inability to confront them, or to let empathy become an excuse we hide behind from our responsibility to provide effective treatment.
And I have a bigger objection to the therapist who imagines that the client reaching tears, or their defenses collapsing in front of him, is sufficient proof that the intervention succeeded.
The collapse of defenses is not, in itself, a therapeutic achievement.
It may be a useful step in an appropriate therapeutic context, and it may be a sign that the therapist has exceeded the client's capacity to bear it.
And the difference is not determined by the client's tears, but by the clinical understanding of what is happening, the person's readiness, their ability to choose, and the means they have to regulate their emotions after the session ends.
The therapy room should not become a place where a person has to defend their defenses in front of the very therapist they came to for help.
I do not want to send out of the room a person who is less solid simply because I succeeded in reaching their fragility.
I want them to leave more able to choose.
To know that they can be strong without staying braced for war all the time, that they can ask for help without losing their independence, and that they can let someone come close without feeling they have lost control of their life.
To have the capacity to confront, and the capacity to rest from confronting.
In the end, the survivor does not need a therapist who is dazzled by their endurance, nor one who sees in them nothing but a victim deserving of pity.
They need someone who knows how to look at the wound without letting it hide the rest of the person.
Someone who respects their history without locking them inside it.
Someone who can say to them:
I see what happened to you, and I also see what you did to still be here.
I will not glorify every way you used, because some of them may have cost you more than you could bear. And I will not demand that you give them up before we understand why you needed them, and how you can live without remaining dependent on them.
I did not come to take from you the weapons you survived with, and then leave you unarmed before the world.
I came to help you know when you need a weapon, when you can set it aside, and when you no longer have to fight a battle at all.
But to confuse healing with stripping the survivor of their strength, and empathy with treating them as an extremely fragile human being, that is not mercy.
And sometimes, when it comes from a therapist who does not realize what he is doing, it becomes another form of harm.
