I read a post by a physician saying that one day we will discover that many of those diagnosed with borderline personality disorder or fibromyalgia are in fact patients with complex, chronic psychological trauma, and that those diagnoses have misled them.
له كل العلم والاحترام، لا أنتقص من أحد ولا من معرفته، لكني أضع بضع تساؤلات…With all due respect for his knowledge, I take nothing away from anyone or from what they know. I only want to raise a few questions.
And because the post has spread widely, I think we need a more precise scientific discussion, not more sensational statements that sound as if they are exposing a truth psychiatry hid for years.
My dear sir, no one denies the profound role psychological trauma plays, especially early childhood trauma, in shaping patterns of emotional response, human relationships, and the sense of self, and even in some of the processes linked to chronic physical pain.
And no one denies that there are patients who received inaccurate diagnoses, or whose trauma history was not examined closely enough.
But let us ask a simple question:
Since when did discovering a possible causal factor, or a shared risk factor, become proof that a diagnosis is invalid?
If we discover that trauma contributed to the development of borderline personality disorder, does that mean borderline personality disorder no longer exists?
And if we know that trauma may affect how the nervous system processes pain, has the diagnosis of fibromyalgia become merely a misleading label?
Here we are confusing three entirely different questions:
What contributed to the development of the disorder?
How does the disorder show up in the person's life?
And which treatment can help them?
These questions, though connected, are not one question.
The cause is not the diagnosis, and the diagnosis is not the treatment plan.
Take, for example, borderline personality disorder and complex post-traumatic stress disorder.
Yes, there is overlap in emotion dysregulation, relationship difficulties, and a negative self-image. But the studies that examined the diagnostic structure of each did not conclude that they are one disorder under two different names.
Rather, they found evidence supporting the distinction between them, while acknowledging overlap and dual diagnosis.
And this is not merely a difference in terminology.
Some patients suffer clear trauma symptoms: re-experiencing the events, avoiding the cues linked to them, and a persistent sense of threat, alongside disturbances in self-regulation.
Others suffer a long-standing pattern of unstable relationships, identity disturbance, impulsivity, intense fear of abandonment, and self-harming behaviors.
And the two pictures may come together in one person.
By what scientific logic do we ask them to choose between the two diagnoses, if they meet the criteria for both?
Moreover, trauma is not a necessary condition for diagnosing borderline personality disorder, just as exposure to trauma does not necessarily lead to borderline personality disorder or complex PTSD.
Turning trauma into the sole explanation for every disorder is not an advance in understanding the human being. It is a new reduction of them, even if it looks more humane than the old one.
As for fibromyalgia, the matter calls for even more caution.
We are talking about a chronic pain syndrome, linked to changes in pain processing and sensitivity, with recognized diagnostic criteria.
It is true that research has found an association between traumatic experiences and some chronic pain syndromes, but association is not proof of causation, and it is no license to cancel the physical diagnosis or reduce it to a psychological disorder.
A patient living with chronic pain does not need a battle between body and psyche to prove that their suffering is real.
The pain is real, whether neurological, psychological, or social factors contributed to it, or all of them interacted together.
Then we come to the matter I consider most important.
Suppose a patient was diagnosed with borderline personality disorder, and a reassessment then revealed that they also suffer from complex PTSD, or that the latter explains a larger share of their symptoms.
Does that mean all the treatment they received was in vain?
Was learning to regulate their emotions a mistake?
Was their stopping self-harm a worthless outcome?
Is their gaining the ability to tolerate distress, build more stable relationships, and reduce life-threatening behaviors a misleading treatment because it began under a different diagnostic heading?
Of course not.
We have scientific evidence for the effectiveness of dialectical behavior therapy (DBT) in reducing borderline symptoms, curbing self-harm, and improving psychological and social functioning, with the strength of the evidence and the results varying across studies.
We also have modified DBT interventions for PTSD, among them DBT-PTSD, which have shown encouraging results in clinical trials with people who suffered severe childhood trauma.
This is not a call to ignore accurate diagnosis, nor to settle for easing symptoms while the trauma goes untreated.
It is a reminder that psychotherapy is not a battle to defend the names of disorders, but scientific and human work aimed at changing patterns of suffering, reducing risk, and restoring the capacity to live.
Certainly, a change in diagnosis may be a necessary reason to adjust the treatment plan and add specialized treatment for traumatic memories and their effects.
But that does not erase the improvement achieved, nor make every earlier intervention a wrong treatment.
Let us also be more precise when we talk about medication.
Medication is not the primary treatment for borderline personality disorder itself according to current professional guidelines, though it may be appropriate for targeted symptoms or co-occurring disorders, subject to medical assessment and ongoing review.
Hence, defending the value of treatment does not mean claiming that every prescription was necessary, or that everyone diagnosed with borderline personality disorder received sound treatment.
Accurate diagnosis matters, but effective treatment matters too, and we must not make one the enemy of the other.
I fully agree with the saying: “A sound diagnosis is the beginning of the right treatment.”
But it is not the whole sentence.
A diagnosis is not a final certificate we hand a person and then close the file on understanding them. It is an organized clinical hypothesis, grounded in criteria and evidence, and it remains open to revision as new information appears.
Nor is treatment an automatic prescription that comes out of a machine once the name of the disorder is entered.
We treat a human being with a history, learning patterns, biological vulnerability, painful experiences, living circumstances, strengths, and goals they want to reach.
This is exactly where the importance of case formulation, functional analysis of behavior, and continuous assessment of treatment response lies.
If we got the diagnosis wrong, it must be revisited.
If we overlooked the trauma, it must be understood and treated.
And if the patient does not improve, we must ask why, instead of insisting on a plan that no longer fits their needs.
But to tell thousands of patients, in a sweeping statement, that they may have been victims of diagnoses that misled them, without offering evidence of the scale of that error or specifying the cases it applies to, is something worth pausing on.
Because those reading these words are not a theoretical audience debating psychiatric classifications.
Among them is a person who spent years trying to understand why their emotions erupt with such force.
Among them is someone who fought long battles with self-harm and suicidal thoughts.
Among them is someone who learned, after years of chaos, to pause for a moment before acting on impulse, to ask for help, and to bear a pain they thought they would not survive.
And among them is someone still at the beginning of the road, trying to build trust in treatment and in the possibility of recovery.
These people deserve to know the whole truth, including the limits of diagnoses and the shortcomings of clinical practice.
But they do not need one exaggerated certainty swapped for another.
My dear sir,
The problem is not that we rethink diagnoses. It is that we present hypotheses worth researching as if they were facts already settled.
And the problem is not that we place trauma at the heart of our understanding of psychological suffering. It is that we make it the only explanation for all suffering.
Science does not advance by cancelling what came before simply because we found a new angle of view, but by re-examining the evidence, correcting errors, and widening our capacity to understand and to treat.
In the end, I want to recall what I consider the most important standard in my clinical work:
Our goal was never for one diagnosis to triumph over another, or for patients to become more skilled at memorizing the names of their disorders.
The goal is for the person to become more able to regulate their emotions, less driven to hurt themselves, more able to build stable relationships, and to find some meaning, freedom, and capacity to choose.
To build a life worth living.
If treatment helped them do that, then a real therapeutic achievement took place, even if new knowledge calls for revisiting our whole understanding of their case.
And if reassessment reveals a trauma that needs specialized treatment, our duty is to add what is missing, not to demolish what was achieved.
A human being is bigger than their diagnosis, yes. But they are also bigger than their trauma.
And we should not reduce them to either one.
There is a problem deeper than the diagnostic error itself.
That we discover a new explanation for human suffering, and think we have discovered the final one.
That we move from treating personality disorders as fixed defects in a person, to treating psychological trauma as the root of every disorder, as if all we had done was swap one diagnostic cage for another, and then call that scientific progress.
I have written before about the danger of the generalization that many of those diagnosed with borderline personality disorder or fibromyalgia are in fact patients with complex trauma, and that those diagnoses have misled them.
I am not here to defend diagnoses as sacred truths, nor to defend wrong clinical practices, nor to deny the misunderstanding, stigma, or neglect of their trauma history that some patients have suffered.
I want to go further than that.
Are we really rethinking psychiatry, or are we reproducing its old mistakes in new vocabulary?
First: When We Confuse the Origin of Suffering With Its Form
If a child is exposed to chronic neglect, violence, rejection, or an environment in which their feelings are not acknowledged, they may develop various patterns of responding to threat, and difficulties in regulating emotion and in their relationship with themselves and with others.
But what will this child become at thirty?
Will they suffer from post-traumatic stress disorder?
Or borderline personality disorder?
Or depression?
Or a substance use disorder?
Or will they meet the criteria for no mental disorder at all, despite everything they lived through?
The scientific answer is that we cannot infer their diagnosis from their trauma history alone.
A traumatic experience does not turn into a mental disorder along a single straight road.
What we know about the interaction between biological vulnerability, early experiences, the social environment, learning, and coping mechanisms makes it hard to reduce these different pathways to one causal story.
In the biosocial theory on which dialectical behavior therapy rests, the invalidating environment was never merely a synonym for trauma, nor was biological vulnerability a secondary detail.
We are talking about a reciprocal interaction between a high emotional vulnerability and environments that may fail to respond adequately to that vulnerability.
That is why the clinical question is not only: what happened to this person?
But also:
What did they learn from what happened?
How do they now respond to threat?
Which behaviors once helped them survive, but today keep their suffering going?
And what needs to change now?
Understanding the past helps us explain the present, but it does not spare us from examining what keeps the problem alive in the present.
This is not a denial of trauma.
It is a refusal to reduce a human being to the moment they were harmed.
Second: Similarity Is Not Sameness, and Correlation Is Not Identity
Take borderline personality disorder and complex post-traumatic stress disorder.
Seen from a distance, we may find severe emotion dysregulation, relationship difficulties, a disturbed sense of self, shame, and perhaps a long history of early suffering in both.
But science does not stop at surface resemblance.
There is a difference between a self-image that is persistently negative, and a self-image that is unstable and shifting.
And there is a difference between withdrawing from relationships and avoiding closeness out of a sense of danger, and living a pattern of intense volatility in relationships, fear of abandonment, and impulsivity.
These are not rigid rules that apply to every patient, but they are research-based distinctions that help us build a more precise differential diagnosis.
A recent systematic review confirmed that the two disorders can be distinguished, despite the high correlation between their symptoms and the possibility of their co-occurring in the same person.
And here I want to raise an important question:
If the two disorders can co-occur, why do we necessarily assume that the presence of one is proof that the diagnosis of the other was wrong?
Is the goal for the complex trauma diagnosis to triumph over the borderline diagnosis?
Or is the goal to understand the patient better?
Differential diagnosis is not a contest to announce a winner. It is an attempt to determine what is actually happening, and what we need to intervene to change.
Of course, we may discover that the earlier diagnosis was wrong.
But that is a conclusion reached by disciplined clinical assessment, not a verdict we settle in advance merely because a history of trauma has come to light.
Third: Has Trauma Become a Substitute Diagnosis for Everything?
Here, precisely, lies the danger of what I call “the explanatory temptation.”
An explanation that can tie a large number of phenomena together looks very attractive, especially when it concerns a moving human concept like trauma.
But the strength of an idea is not measured by how many phenomena we can tell a coherent story about using it.
It is measured by its ability to withstand testing, to discriminate between cases, and to make predictions that can be checked.
If I say trauma may contribute to emotion dysregulation, that is a claim that can be studied.
If I say it may contribute to some cases of chronic pain, that too is a claim worth researching.
But if every emotional disturbance becomes evidence of trauma, every chronic bodily pain an expression of trauma, and every relationship difficulty a direct result of trauma, then we have lost the ability to tell different phenomena apart.
An explanation that stretches to cover everything, with no clear limits, may lose much of its explanatory value.
We do not need to belittle trauma in order to refuse turning it into a ready-made answer.
Perhaps the best way to respect traumatic suffering is not to attribute to it what research has not established.
Fourth: When We Reduce the Body's Pain to the Psyche's History
The inclusion of fibromyalgia in this discussion deserves a pause of its own.
Fibromyalgia is not merely sadness that turned into pain, nor a label doctors invented because they failed to uncover the patient's trauma history.
It is a chronic pain syndrome with recognized clinical criteria, studied in the context of complex mechanisms of pain processing and regulation, alongside multiple psychological, social, and biological factors.
It is true that studies have found an association between post-traumatic stress disorder and fibromyalgia, and that trauma may contribute to greater symptom severity in some patients.
But the existence of that association does not mean fibromyalgia is not fibromyalgia.
More importantly, the diagnostic criteria revised in 2016 do not require the absence of all other disorders for the diagnosis to be valid.
That is, the presence of a co-occurring psychological or medical problem does not automatically invalidate a diagnosis of fibromyalgia.
Chronic pain patients have long suffered from doubt cast on the reality of their suffering. Our legitimate concern with trauma must not become another form of doubting their pain.
It is a mistake to stage a battle between body and psyche, and to hand victory to one by erasing the other.
Fifth: Is Treating Symptoms Worth Less Than Treating Causes?
Here I want to pause for a moment.
Because this question carries a common assumption in therapeutic discourse: that treatment aimed at causes is necessarily deeper than treatment aimed at symptoms or behaviors.
But clinical reality is more complicated.
Imagine a person exposed to repeated trauma in childhood.
When they arrived in therapy, their life was full of self-harm, impulsive episodes, unstable relationships, and one crisis after another.
They began to learn the skills of emotion regulation, distress tolerance, mindfulness, and interpersonal effectiveness.
They learned to notice their impulse without automatically acting on it.
To understand the function the harmful behavior serves.
To look for alternatives that do not buy momentary relief at the cost of their life and future.
To ask for help instead of hurting themselves.
After a while, the number of crises began to fall, and their capacity grew: to work, to build relationships, and to bear feelings they once thought would kill them.
Then a later assessment revealed a trauma disorder requiring trauma-focused treatment.
Does that mean what came before was not real treatment?
To say so seems neither fair nor scientifically accurate.
But on the other hand, does improved emotion regulation mean we have actually treated the traumatic memories, the re-experiencing, the avoidance, and the persistent sense of threat?
Of course not.
And here lies the dialectic we need: the earlier treatment can have succeeded in its goals, and at the same time be insufficient to treat every aspect of the disorder.
Acknowledging the improvement does not cancel the need to complete the treatment.
Just as discovering a new treatment need does not cancel the value of what was achieved before.
A randomized trial published in JAMA Psychiatry in 2020 showed that DBT-PTSD, a modified intervention for trauma, produced a meaningful improvement in PTSD symptoms related to childhood abuse.
The scientific message was not that we should ignore trauma, nor that skills training alone suffices for all patients.
It was that we can develop interventions that integrate treating the effects of trauma with understanding the dysregulation of emotion and behavior, instead of treating them as two separate worlds.
Good treatment does not ask the patient to choose which part of their suffering deserves to be acknowledged.
Sixth: A Diagnosis Is Not a Sacred Truth, but Not a Personal Opinion Either
Psychiatric diagnoses are not separate natural objects we can see directly under a microscope.
They are clinical classifications based on patterns of symptoms, their duration, severity, and effect on functioning, and they are revised as evidence and criteria evolve.
That is why the boundaries of a disorder may shift, a new disorder may be added, or the tools of assessment and intervention may develop.
This is not proof that psychiatry itself has failed.
It may be proof that it can correct itself.
But admitting that diagnoses are open to revision does not mean they are arbitrary, nor that any new explanation can cancel them without being tested.
The same scientific evidence that allows us to revise diagnoses obliges us not to rush to declare them void.
And if we criticize overconfidence in the old diagnoses, we must not fall into the same overconfidence when we speak of the new ones.
Not everything that looks more modern is more correct, and not everything that sounds more compassionate is more accurate.
Seventh: Where Is the Human Being in All These Battles?
Sometimes I feel that public discourse on mental health has become more preoccupied with classification battles than with the lives of patients themselves.
Is this a personality disorder?
Or a trauma disorder?
Or a neurological disorder?
Or a normal response to abnormal circumstances?
These are important questions, and I take none of them lightly.
But I want to add another question, perhaps a humbler one, and one closer to clinical practice:
What do we do with this person's suffering today?
What do we do if they are hurting themselves tonight?
What do we do if they cannot work because of the pain?
What do we do if they are afraid to sleep because their memories hunt them?
What do we do if they fully understand the reasons for their behavior, yet still cannot change it?
Do we ask them to wait until the academic debate over diagnostic boundaries is settled?
Or do we start with what we can help them with, while continuing to assess and to reformulate our understanding and the plan whenever needed?
Clinical responsibility does not allow us to choose between understanding and acting.
We have to do both.
Finally: On the Moral Responsibility of the Medical Word
When a well-known specialist writes an opinion followed by thousands of people, the matter goes beyond the freedom to voice a scientific hypothesis.
He is speaking to an audience that may include patients who are genuinely suffering, some of them rebuilding their trust in treatment after years of failure or disappointment.
That is why distinguishing between “may happen,” “happens often,” and “scientifically established” becomes part of professional responsibility.
I am not asking anyone to stop criticizing psychiatry.
On the contrary, we need bolder criticism, especially of diagnostic stigma, the neglect of trauma, the unjustified overuse of medication, and the treatment of patients as mere clusters of symptoms.
But I ask that the criticism meet the same standard of responsibility we demand in diagnosis and treatment.
It is not fair to criticize the haste with which patients are given a diagnosis, and then to hasten ourselves to tell them their diagnosis was wrong.
And it is not fair to blame the therapist for reducing a person to their disorder, and then to reduce the person ourselves to their traumas.
My dear sir,
The humanity of psychiatry does not lie in choosing the explanation that stirs the most sympathy, but in choosing the understanding most faithful to the evidence, even when it is complicated and uncomfortable.
The patient may have borderline personality disorder.
They may have complex post-traumatic stress disorder.
They may suffer from both together.
We may indeed have misdiagnosed them.
We may have succeeded in easing one side of their suffering while failing to notice other sides.
All of these possibilities deserve examination.
What we must not do is turn one of them into a general truth, and then ask patients to reinterpret years of their lives and their treatment in its light.
A human being is not their diagnostic label, not the sum of their painful memories, and not only the behavior we are trying to change.
They are also their capacity to learn, to build new meaning, and to regain choice after years of losing it.
Perhaps one of the most dangerous consequences of trauma is that it convinced the person that what happened to them will keep determining everything they will become.
Should we, while trying to help them, convince them of the very same idea, only in more scientific language?
Understand the trauma, revisit the diagnosis, correct the errors of treatment, but do not reduce the human being all over again.

A human being is bigger than their diagnosis, and bigger than their trauma.
At Life Ark we do not treat a diagnostic label. We start with a clinical formulation of your whole case: your history, what you learned from it, and what keeps your suffering alive today. Then we work with the tools of DBT on what can change now, revisiting the understanding and the plan whenever needed.
